Benign Prostate Hyperplasia

The prostatic gland is an exocrine gland found only in the male. Exocrine glands secrete chemicals to the outside of the body. It is a glandular organ and is found just below the bladder and at the top of the penis. It is normally about the size of a walnut and it surrounds the urethra which is the tube that brings peeing from the bladder to the outside of the body. It is made of fibrous tissue and some muscle tissue. Anatomy The gland is divided into zones or lobes. The peripheral zone is the outside dissipate of the prostatic and this is where most prostate cancers are found.The central zone makes up about 25% of the gland and the transition zone makes up about 5% of the gland. The transition zone, however, is where are the hypertrophy happens. This part of the prostate is closest to the urethra so as it grows it puts pressure on and squeezes the urethra causing difficultys with urination. However, in benign prostate hyperplasia, the prostate grows and begins to cause the per son problems. It is usually an time related disease which is nonmalignant. Large lesions grow on that central section of the prostate.It is believed that 60% of men everywhere the age of 60 have BPH (Porth & Matfin, 2007). The exact cause is unknown but there are some risk factors which include age, history, race, dietary fat, and hormonal factors. The ripened one gets, the more likely to have it and African American men get it most often. When young the gland grows with the hormones testosterone and dihydrotestosterone and it is believed that in some men the hormones do not slow down and stop when they should so the gland continues to grow past what would be normal. Signs and SymptomsBPH causes compression on the urethra so it causes partial or nail down obstruction of urine flow. As the obstruction increases (prostate gets larger), acute or emergent retention of the urine may occur with distention of the bladder. The urine that is left in the bladder when a man tries to urinat e and is not able to empty his bladder causes a frequency to occur, consistently trying to empty the bladder. This usually becomes worse at night. When there is a lot of bladder distention accidents may happen every time pressure is put on the abdomen or when coughing causing pressure inside the abdomen.When urine is not completely emptied from the bladder the chances of a urinary infection occurs and this sometimes happens frequently. All of this develops in stages over time so there is often changes in the kidney and ureters because of the long time back up of urine causing hydronephosis which eventually begins to kill kidney gomeruli. If not set early this can lead to renal failure. Diagnosis When making a diagnosis of BPH seven questions are at first ask. These questions are regarding rudimentary emptying, frequency, intermittency, urgency, weak stream, straining, and nocturia.A final question relates to the quality of life the patient is having related to this problem. A his tory, physical exam and rectal exam is make. Also to be through are urinalysis, PSA, and serum creatinine as well as other blood tests. The urinalysis is done to detect bacteria or infection, the serum creatinine is used to deem whether there is renal function problems and the PSA is used to screen for cancer. Our Patient JJ has BPH and he has marks of frequent or urgent need to urinate, he too has developed cysts in his urine and a urinary tract infection.All of these symptoms make sense because they are symptoms of the enlarged prostate putting pressure on the urethra. The urinary tract infection has occurred because of the retention of urine in the bladder. His sermon plan is medications and TURP or TIP. Treatment Treatment in the early stages is aimed at symptom relief, then there is a time of watchful waiting which is suggested by most physicians. There are presently medications that work pretty well for some time. These are Finasteride which reduces prostate size by bloc king the effect of androgens on the prostate.This drug also cause atrophy of the prostate cells which can cause a 20% reduction in volume. The onset of the drug takes some time, sometimes 3-6 months but it usually works for some time. The side effects may be erectile dysfunction and decreased libido. There can also be the surgical removal of the enlarged prostate gland. This surgery can be done through the urethra, through a suprapubic incision, or through an incision in the peritoneum. The most commonly used way is called a TURP which is transurethral prostatectomy or through the urethra.The prostate is a very vascular organ and though this procedure is fairly simple to do, there is a great deal of bleeding on average and irrigations are often necessary post-operatively. There are presently many experimental treatments that it is hoped will provide less side effects. These include laser surgery, transurethral vaporization, transurethral microwave therapy, and transurethral needle a blation. There is also a new technique using balloon dilation but these effects at this time bet to be transitory. Stents can also be placed in those men that have major heart of lung issues and cannot tolerate surgical procedures.Treatment Plan So, JJ had symptoms of BPH which was treated by medication but his symptoms persisted and he had a TURP. This procedure after recovery relieved his symptoms and he is progressing quite well with most of his symptomology gone at this time. Conclusion BPH which is thought of as a cancerous condition of the prostate is a benign condition of the prostate. The symptoms which it causes are usually the method of treatment in the beginning and this may last for some time, but there may be a later need for a more ravening method of treatment which at this time is the TURP.There are side effects of this surgery, some of them immediate because of the vascular condition of the prostate. This surgery, at this time usually solves the problem long term. There are many more less invasive procedures being studied at this time. Our patient JJ is doing quite well after a TURP and will most likely not need a further procedure. Character Education and Core Virtues Essay

When I signed on to teach English at a core virtues discipline, I had no idea what I was in for. I nodded and smiled in my interview when the Headmaster explained the virtues curriculum, and I parried back with everything I thought she wanted to hear how I could infuse my lessons on To Kill a Mockingbird with discussions about empathy and courage. I may have even quoted Atticus line about traveling around in someone elses skin. I figured I could tack on some of that quaint virtue stuff before getting to the real meat of the lesson, the academic stuff.And for the first form I taught at Crossroads Academy, thats pretty much what I did. I made some empty gesticulations toward the core virtues bulletin board in my classroom and made some token mentions of fortitude at obvious moments in our reading of The Illiad and The Aeneid. I was teaching literature, but I certainly wasnt doing Aristotle proud. I convey come on. Character direction? Core virtues? I teach English, not Sunday scho ol, and besides, I teach nerve center school.If I were to walk into my eighth grade English class and wax rhapsodic about prudence and temperance, those kids would eat me alive. Its hard enough to keep the attention of a classroom full of middle school students without coming on like an 18th-century schoolmarm. Character education is not old-fashioned, and its not about bringing piety in to the classroom. Character is the X instrument that experts in parenting and education have deemed integral to success.Somewhere along the way, someone must have started dosing me with the character education Kool-Aid, because five years in, I have come to understand what real character education looks like and what it can do for children. I cant imagine teaching in a school that does not have a hard-core commitment to character education, because Ive seen what that education can mean to a childs emotional, moral, and intellectual development. Schools that teach character education spread abroa d higher academic performance, improved attendance, reduced violence, fewer disciplinary issues, decrease in substance abuse, and less vandalism.At a time when parents and teachers are concerned about school violence, it is worth noting that students who attend character education schools report feeling safer because they know their fellow students value respect, responsibility, compassion and hard work. From a practical perspective, its simply easier to teach children who can exercise patience, self-control, and diligence, even when they would earlier be playing outside especially whenthey would rather be playing outside. American schools used to focus on character education and civic virtue. The founders of this country, including washbowl and Abigail Adams, Thomas Jefferson, James Madison, and Benjamin Franklin wrote about the importance of character education in maintaining the new republic. Those founders would likely be horrified by the loss of this goal, as they all cite character education as the way to create an educated and virtuous citizenry.As Gallup polls base that over ninety percent of American adults support the teaching of honesty, democracy, acceptance of people of different races and ethnic backgrounds, patriotism, caring for friends and family members, moral courage, and the Golden Rule in public schools, it seems odd that this expression of American education has disappeared from public debate over curriculum and academic content. The core virtues prudence, temperance, fortitude, and andice introduce it into nearly every lesson we teach at our school and every facet of our daily lives on campus.The curriculum we use, designed by Mary Beth Klee, is a non-sectarian education in intellectual, moral, and civic virtues through literature, and can be used in conjunction with any academic curriculum. As the core virtues program uses examples to literature in order to illustrate character, I choose my texts accordingly. In my middle sch ool Latin and English classes, we explore the concept of temperance through discussions of Achilles impulsive rages, King Ozymandias petulant demand that we Look on my works, ye Mighty, and despair, Macbeths bloody, vaulting ambition, which oerleaps itself and falls on the other. This calendar week, I gained a fantastic teaching assistant who has raised my character education skills to the next level, a immaterial teacher who has illustrated the importance of temperance far better than I or Achilles or Macbeth ever could. Her first day in residence, she spent more time off the eggs than she did on them, and we realized we were going to have to find a way to teach our students some self-control.It just so happens that this months virtue is temperance stopping to think about our actions before we enact them, giving the best of ourselves, and saying no to our weaknesses. The middle school students use the term temperance, and the lower school kids use the term self-control, but tomato, tomato, its all the same idea. In Stanfords famous sample on self-control, children were faced with the immediate reality of one marshmallow versus the promise of two marshmallows if they can just delay for fifteen minutes.The children who were able to resist temptation and wait fifteen minutes for that second marshmallow had better life outcomes in the form of lower obesity rates, higher SAT scores, and higher levels of education. Self-control itself does not make a kid smarter, or fitter, or more proficient at test-taking, but its the essential skill hidden within all of these coercive outcomes. Character education is not old-fashioned, and its not about bringing religion in to the classroom. Character education teaches children how to make wise decisions and act on them.Character is the X factor that experts in parenting and education have deemed integral to success, both in school and in life. Paul Tough, author of How Children Succeed, calls that character-based X factor grit, while educational consultant Dr. Michele Borba calls it moral intelligence. When I asked parenting expert Borba to explain why she thinks character education is so overlooked as a lively part of children s success, she wrote, Thats what parents dont seem to get, the hidden values of character traits for success. They see character education as fluff, because thats frequently how its taught posters and worksheets.Character education needs to be relevant. It needs to be woven in curriculum, not tacked on. We are such a trophy-, SAT-obsessed society, but if parents would cognise the value beyond the humanness, civility and ethics, they might get it. Here on our campus, our marshmallow is a duck. Our students must weigh their desire for a quick peek at Mom Mallard with the promise of ten ducklings waddling around our playground in 28 days. We celebrate the Christmas because the birth of savior Christ gives us new hope and newlife. The births of deliverer Christ on the manger show and reflect the humility of the Son of God. Also through this humility of the son of GOD, the three wise men give homage, respect and gave different kind of gifts that only for a king. The birth of Jesus Christ rejoices our heart in gladness. The birth of Jesus Christ is knocking at a door of our hardened heart and dogma us to be a child again. A c hild heart that is full of hope, happiness, humility, care, love and faith. Its amazing how God really works and show how much He care and love the human kind through his Son Jesus. God sent his son Jesus to the world to carry a greatest mission, a mission to save all mankind from sin that weaken and destroy our faith. Jesus lives a life on earth with simplicity, love and obedience to his father Joseph and mother Mary but He never forget the greatest work that our Lord God put on his shoulders. He started to proclaim the words of the Lord.He started to travelled and performed different kind of miracles. He touches and changed the life of different people. He healed different kind of sickness that makes our earthly body weak. Jesus Christ founded the sacrament of the Eucharist. duration they were eating, Jesus took bread, said the blessing, broke it, and giving it to his disciples said, Take and eat this is my body. Then He took a cup, gave thank and gave it to them saying, Drink fr om it, all of you, for this is my blood of the covenant, which will be shed on behalf of many for the forgiveness of sins. (Matthew 26 26-28). At the last patronage Jesus founded the Eucharist to share with us His life and to provide us with the commemoration of his suffering and pain. When the time of Jesus Christ came, He carries his greatest mission.He shows his humility, as his theology and divinity are trampled upon by the people he has chosen to be his own. People spit in His face and He struck him and while other slapped him by saying that his not the Messiah. But Jesus Christ accepted every pain, mocking and humiliation that the people gave to him without talking back or complaining. Jesus Christ can ask God the Father to send angels and protect him but he chooses to suffer in obedience to the will of the Father. Jesus Christ embraced all mankind, when He stretched his hands on the elude to redeem us from our sins. Jesus Christ cross proclaims the wonder of his love to al l of us.God the father willed his one and only begotten son Jesus, to die on the cross for the sins of the world for us to insure how much God and Jesus Christ love the mankindunconditionally. As we accept Jesus Christ, current God and true man in our life, his victory on the cross teaches us to reflect on his suffering, death and resurrection in order for us to have an unfailing faith in Thy goodness that has no end. Jesus Christ led us to many beautiful lessons. He teaches us to reflect and to be strong and to hold on to our faith in the midst of present danger and fear. He teaches us to bear our cross so that we may be united to his suffering as we pass by on our earthly life and to be worthy to hope for a new life in the kingdom of God.

And when they are not sure about their career and could not plan much for it, they might get de- motivated which is again not a very good sign for the company. Despite having the formal procedures the company is utilize informal procedures and given employees full freedom so that innovation may continuously take pop in the company. But on the same time it is a growing company with lots of potential. shortly it has fifty seven thousand employees which are expected to increase so such an informal and flexible culture would never sustain in a growing environment. Recommendations Hp is a huge organization with about 57000 employees all together.The companys culture has been working fine for the employees who get involved in it and with time, they do become a part of it. In its culture, employees were given freedom of how to complete their tasks in a given time along with some set of procedures they had to comply with. This culture has been followed since the companys long history and has proven to be successful and an edge over the other companies. To keep up with this culture and also to make new comers feel comfortable and see themselves as adjustable to it, we specify that here, the fictional character of the senior employees comes into part.The seniors have to be more compliant, more welcoming and help the new employees feel that now they are a part of it. Another problem we see in the case is a disfigurement in the Human Resource Departments plan. As mentioned in the case that employees left the company in an effort to start their own ventures and when they failed with it and cute to join Hp again, they were welcomed in the company. First of all, there shouldnt be employees who leave the company. Reasons for this should be identified and taken into notice.A reason mentioned in the case is that there were no defined career paths for the employees which could be a major reason for employees in departure Hp. This thing should be resolved by proper season planning and giving a defined path to the employees so that they get to know what position in the firm they might be aiming for in future. Secondly, the employees who wanted to rejoin Hp must not be welcomed to rejoin. This would create a norm within the employees of leaving and coming back whenever they wish to.If they have this idea in mind that it is safe to leave their job at any point in time and if they come back theyll be accepted, they would work half-heartedly for Hp and not perform as they are capable of. So, we recommend that a fair of job security should be there in the e mployees before they decide to leave the firm. With a proper succession plan, the posts of the employees who leave should be filled instantly and the post would be no longer available for that employee who left. This way, the employees would think twice before leaving the firm and then make their final decision.
